Fluid=damage: Swelling within the knee joint is most often a sign of structural damage. Cartilage injury(both meniscus and articular cartilage) problems would be the most likely source of swelling. Patellar (kneecap) instability, tendonitis (if swelling outside the joint) and chronic ligament tearing are other possibilities.
