Medical class III: I'm a private pilot as well as MD. You should easily pass FAA's basic (Class III) exam, if your overweight problem doesn't limit mobility. More fail because certain medications aren't approved or they have an alcohol or other substance abuse problem rather than other health reasons. With new FAA regs soon to come, anyone with a valid driver's license can self certify with no medical exam at all.
