Unlikely: Diabetic nerve pain usually starts as a numbness or tingling, not sharp pain. More likely strain or tendonitis.

Nerve: A pain that shoots down from the elbow to the wrist and only lasts a few seconds is most likely related to nerve entrapment or irritation at the elbow. Diabetic nerve pain would be constant and would start in the hand. Occasionally elbow arthritis pain can radiate down the arm but it lasts much longer. See your primary care physician if it persists more than a week.
