I have been waking up lately with my thumb and first two fingers swollen with blood, when i extend and raise my arm the swelling is relieved.

Circulation.: I am assuming you mean swelling from excess blood flow and not actual blood. Could be a DVT in the arm. It would be important to have an exam done and maybe some testing.
