Time to see an MD: What is causing this is unclear. Though there are many possible explanations the one most worrisome would be a cancer. This is even more of a concern If you have smoked, or if you have ever had a habit of using chewing tobacco. Of course there are other things that are possible, and usually cancer is painless, but I suggest getting it checked out by a doc in person ASAP.

Get checked : These spots may be due to an infectious or autoimmune process or reaction to something you put into your mouth. You should get examined soon by your doctor.

See your Dentist: Sublingual (under the tongue) tissues often appear purplish-red because of blood vessels near the surface. Peeling tissue is, of course, not normal. Unlike Physicians who look past the mouth into the throat when doing an examination, Dentists focus on the mouth. Please see your Dentist who can perform differential diagnosis, treatment (if necessary), or referral to appropriate MD/DO or DDS/DMD.
