Stop the COCAINE use: Detox is of the first order, see your primary care physician initially, you may also need to be evaluated by a cardiologist (heart doc). Cocaine damages the heart Once you are drug free and heart healthy your primary care physician can refer you to an Ear-Nose & Throat (ENT) physician to have the septum repaired. Probably depending on the degree of septal damage surgery may be in your future.

Stop cocaine, get help!!: This requires severe and continuous cocaine use, which I am sure is causing many more problems in addition to your septal perforation. An otolaryngologist (ENT doctor) can assess and potentially treat the perforation, which may require a cartilage graft. Untreated, the nose may actually collapse. This level of cocaine use requires intensive treatment, probably inpatient, as well as counseling.
