Need MRI LS spine: If by L5-S1 pain you refer to low back in that dermatomal area this could be a from something near those roots pressing on those nerve roots (that something could be either the disk or fat tissue or a nerve or a connective tissue tumor and MRI L-S spine with contrast is the best study to look for the cause
