Itraconazole instead: While fluconazole for candidiasis is frequently utilized; perhaps, you might consider itraconazole as a better fit for treating fungal infections such as ringworm. Both medications have precise pediatric dosings listed in the immensely useful and free app "Epocrates" easily downloaded to your smart phone or iPad.

Varies: Harriet Lane lists only dosing for invasive candidiasis at 6-12mg/kd loading dose with 3-12mg/kg daily maintainance for these infections. Max 12mg/kg/dose. The Red book describes a loading dose followed by once weekly dosing for T.pedis, but lists no dosing recommendations.A similar citation is listed for T.capitis.
