Could be: Sometimes there are underlying conditions that allow overgrowth of the candida fungus. See your dentist or pediatrician for examination asap.

Needs to be seen: It could be a number of things, some benign and some not. Another possibility is something known as geographic tongue. See the adjacent photo. In any case, take your son to a general dentist or an oral surgeon for a definitive diagnosis. He\she will tell you exactly what it is and if any treatment is needed. It is always better to be safe than sorry.
