Could be serious: If he uses much alcohol or drugs but not as much recently, it could be withdrawal (which would be put off ---delayed---by drinking or using again) but it could also be other, even more serious things. He should take his temperature and you should be sure he gets seen by a clinician. No one has these symptoms unless something is seriously wrong. Good luck.
