Can you rupture an appendix without knowing?

No: Appendix rupture only occurs with acute inflammation. With rare exceptions you will know if you have appendicitis and especially if it ruptures. For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. Drink enough water daily so that your urine is mostly colorless. Practice safe sex.

Depends. : A free rupture into ones abdomen can cause sepsis and multi organ failure within hours to days. Most ruptures though are contained between loops of intestine or fat and can smolder for days before spreading . Either way you usually have severe pain. I have found some patients with perforations who just thought they had bad constipation or stomach aches
