Many things: It could be from uterus, ovary, appendix or colon. If you have fever and nausea then that could be appendicitis so you should be seen by a doctor immediately. If the pain is severe then see a doctor as soon as possible to get an exam and testing to sort this out.

Possibly: menstrual or uterine/pelvic pain or spasm due to hormonal influences, inflammation or infection. Other possibilities would be uterine fibroids, ovarian cysts or disease and endometriosis. See you GYN for a transvaginal ultrasound.
