Bad cavity: Any cavity is a bad cavity. It's an infection that requires immediate treatment. Delay will only allow it to spread to other tissues. The fact that you are symptomatic indicates that the infection has progressed significantly. Please do not wait. See your Dentist as quickly as you can.

How to know: The way to know what's going on, the cause of the pain and what treatment is required is to see a dentist for a clinical exam and x-rays. The little black dot may not even be the cause of the pain. What appears small is often not and what patients fear as "bad" ,may not be. You may even have more that one issue. See a dentist and find out.
