Cough/rhinorrhea and: fever in a toddler is usually caused by a viral infection. Use of OTC cough medication is discouraged. Supportive measures provide relief. 18 mth old with fever & increasing nocturnal cough deserves an evaluation by his pediatrician to screen out treatable conditions / confirm a viral etiology. Viral illness : Combination of Fever , cough and runny nose are due to viral infection . Cough mostly gets worse at night . If nose is blocked please use saline drops for nose .if he gets wheezy or has rashes , or not wetting nappies then please see doctor.
