What you describe: is not uncommon, I believe. The routine associated with work will often include meal planning, exercise, etc. So when you retire, the whole thing can fall apart. Do an inventory and identify what parts of your old routine worked well for you: you enjoyed it, it didn't cause injury, etc. Then redesign your healthy program one step at a time; chance of failure rises if you try to do all at once.
