My girlfriend woke up with a large area (2-3 inches wide) on her leg raised, swollen, hot to the touch and hard like a tensed muscle. what caused this?

Skin infection: Could be a skin infection called cellulitis or possibly an insect bite. Hard to know without seeing it. It would be best if she saw a doctor.

Swollen area: What you describe sounds like either cellulitis (a bacterial infection of the skin), or perhaps a brown recluse spider bite. Please see a doctor immediately.
