What would happen if someone on warfarin for something they can not discontinue(afib, artificial valve, etc) needed major surgery? can you even do it?

Yes: This should be done in consultation with your doctor. Warfarin can be suspended for surgery. If need be coverage may induced with short acting anti-coagulant like heparin.
