4 year old shin injury still hurts could it be a blood clot?

Unlikely: Blood clots usually cause acute pain and swelling. An injury years ago that still hurts is likely not related to a clot. Shin splints are a more likely explanation.

Calf pain squats: Calf pain can occur from squatting or kneeling. The foreleg has two bones, the tibia and fibula. Fibula's head on the outer side below the knee can get jammed backwards (usually) from squatting, tensing the leg through the ankle. Chronic or extensive bicycling and running/jogging/walking can hurt long term -- check technique and gear. Scars & compartment binding can persist. Not a blood clot.
