Maybe: It is possible to have injured the vulva or vagina with a fingernail in the process of taking the tampon out. If that is not the case, see a GYN MD if the bleeding continues for evaluation.

Yes: If bleeding persists, see a doctor immediately. It is possible as you can cause trauma to the lining inside the vagina using tampons.
