Dizziness for 6 months. gets worse with head movements, sometimes vertigo. plus tinnitus, hyperacusis and vision problems. mri fine. any advice?

Likely central: Hi Joe, dizziness with visual acuity changes and hyperacusis may point to a central problem rather than a problem with your inner ear. One of the more common conditions associated with this constellation of symptoms is migraine variant. I'd consider obtaining vestibular testing and if negative, a neurologic evaluation.
