Ongoing issues: There are several reasons that could be happening... Trapped food in the teeth, dental decay, post nasal drip as seen in allergy, or even esophageal reflux. Proper flossing and using a good mouthwash may address some of the question, but ask if you are a mouth-breather while sleeping. Dry mouth can also lead to smells.

Halitosis: Just brushing alone is not good. One has to floss as well. Given that you are from india I have to ask if you chew "paan" or tobacco? Both of these are also bad for you. Carry a mouth rinse and use this at least 4/day. Gargle may also help. Another cause is dental caries/cavities.

Brushing gets only: Some of the food particles; it usually doesn't get between the teeth well; so flossing is needed to remove those particles. More can be lodged among the papillae (taste buds) of the tongue; so brushing or scraping the tongue is needed to remove them. Other causes: gingivitis; tooth decay; dental abscess; infected tonsils; post-nasal drainage; sinus drainage. See dentist/ent if problem persists.
