This depends a bit: on the circumstances and the symptoms. Trained athletes are fine with a heart rate in the 30s whereas other people are hypotensive, in shock and need immediate intervention. Some patients walk into the ER with a heart rate of 200 while others pass out. In most normal individuals less than 40 and greater than 180 would be concerning.
