Either of two: Alcohol is toxic to brain and cause tissue loss. So can brain injury. It may be a combination of these two insults.

CerbeelarAlcoholeffe: I commend you for not using Alcohol now. Structural damage in the cerebellum of alcoholics is tissue volume loss in the anterior superior vermis due to either shrinkage or atrophy of Purkinje cells (large nerve cells that make up mu much of the volume of the vermis).
