Impressions: A focal pupillary dilatation, may merely be associated with a benign familial condition termed Adie's pupil, and rarely is seen with migraine autonomic dysfunction. Biggest question is whether it fluctuates, or whether it is permanent and worsening, as rarely this may be a sign of pressure on the 3rd cranial nerve. Either neurologist or ophthalmologist can help pin this down.
