Not much: There is nothing that we know of that should prevent one from flying after brain surgery even if you had to do it immediately after the surgery. The pressure inside of the airplanes are so well controlled that the change in pressure should be minimal. However, flying is a tiring event even for someone who did not have brain surgery. Therefore, you will be more tired and and hurt more when you land
