Yes: A mass of this size is always of concern even it may just be a lobe of the thyroid. Do get the ultrasound.

Possible tumor: Any 4 cm lesion in neck is a tumor Even though asymptomatic, origin and treatment needed. The most common growth would b a thyroid tumor followed by lymph node especially if along jugular chain. CAT scan of neck helpful followed by resection. If node, full dx not perfect on needle aspiration, therefore excision for dx is best. If thyroid or nodal Ca in origin further treatment may be necessary
