After a while: The symptoms can persist for a couple weeks after treatment. Sometimes it takes more than one treatment, but even with killing all the bugs, it will take a couple weeks for the rash to resolve.

Yes, it's contagious: Jock itch is a fungal infection and can spread to other parts of the body or to other people. Treatment is usually with over-the-counter anti-fungal creams. Rashes that don't go away easily, or those that keep returning after treatment, should be evaluated by a doctor. People with diabetes get more fungal infections, and have a harder time getting rid of infections.

Maybe.: Interesting question. Staphylococcus infection of eczematous skin is certainly associated with increased itchiness. But its not clear if the bacteria increases itchiness or simply exploits the skin injured by scratching to cause a secondary infection.

Antihistamines: For most patients antihistamines such as benadryl, (diphenhydramine) zyrtec, Allegra and Claritin are effective. If these are not effective second line treatment (off-label) could include Doxepin and singulair and h2 antihistamines, such as zantac. It is important to find the cause of the hives, and an allergist may be able to help.
