Dentist, HELP!!: Good day; Between an excellent dentist and capable internist( Gastroenterologist) your health issues should improve. In the mean time there are potent mouthwashes and sprays(OTC) that will mask your Halitosis!!

Horrible breath: A consult with a competent Periodontist, ENT specialist and Gastroenterologist should be able to diagnose this issue and offer help. If you can't find answers with the one's you have tried, I suggest you try a dental department at a teaching hospital in your area.

Halitophobia: also known as delusional bad breath. You may suffer from halitophobia if you go to all above doctors and they tell you there is no problem but you still believe you have horrible mouth and nose breath. See psychologist for CBT.
