Shoe misfit: If thy shoe offends thee, take it off and get one that fits. The fit of a shoe is from the heel to the widest part of your foot signaled by the base of the big toe and definitely NOT there your toes are. If the shoe clerk grasps the end of the toe box to check an accurate fit get a more seasoned and knowledgeable clerk to check your size on a metal measuring device, not the wooden stick.

Have you tried : Wearing different shoes? If gout was ruled out, maybe you have a dorsal spur? Was n X-ray taken? Maybe there is a functional hallux limitis? It appears the issue is shoe related or exacerbated,,,,,

Nerve.: Not sure if the pain is "in" or "on" the toe. I am assuming that x-rays were taken and they were negative. I would consider that a nerve branch is being affected. Sequential nerve blocks could find the route delivering the pain signal. From there is is easier to isolate the exact site of the pain source. It may take a few visits, but can be used to find the triggering site.

Varies: There are a number of possibilities, I would not hesitate to see a foot specialist. The idea that it f goes away after shoe removed suggests shoe could potentially be aggravating it. Is forefoot of your shoe large enough? Is it compressing forefoot? Neuroma, bunion, callus, corn and multiple other potential cause. Detailed exam and studies likely to determine cause.

ARTHRITIS: A little more information, such as getting better/worse, sudden onset or gradual, aggravated by anything other than shoes, type of job, any OTC analgesics help. Does the big toe move, or is it limited? Trauma? Only one foot? Does it hurt to move it? It sounds like a set of x-rays would be in order, to eliminate arthritis in the joint. Regards, Dr. Latva

Gout: If the pain goes away after taking shoes off it is likely to be gout. could be because of bad foot ware and you may need to see a orthotics. Poorly fitted shoes could be the cause and repeated trauma may be the trigger. your one standing work hours will aggravate the situation and conditions like bunnies, blisters, corns ,arthritis and calluses can be other reasons for the pain.

Great tie pain : Can’ be something from as simple as Poorly fitting shoes but also other conditions such as Hallux rigididus or Turf toe or a neuroma or a bunion or a adjacent stiffnor Hammertoe Can throw Off your gait with shoes on see a podiatrist or An ortho foot specialist
