Right sided: If discomfort is quite localised to the right lower quadrant of the abdomen and especially if there's associated tenderness in that area, I would be concerned about appendicitis or right sided diverticulitis in view of accompanying fever. A viral intestinal infection is a less likely cause of these symptoms but could be the problem. However I recommend that you be examined tonight in the ER.
