EVALUATION: Get an appointment with your PCP for an evaluation. Increase fiber intake,adequate fluids. Watch what you eat as some foods can cause this

Need an exam: You need to see a physician to properly diagnose your problem. There are many possible causes for your symptoms and just giving you a list won't be helpful. You need to discuss your symptoms in person with your physician who will then examen you. Don't try to self diagnose this type of problem.
