The white blood ...: count (WBC) is usually elevated in appendicitis. However, sometimes it isn't. In that case, there is usually at least a left shift to the cell differential. That means that there are nore acute inflammatory white blood cells (leukocytes) than normal (i.e., more neutrophils and bands, fewer lymphocytes). It is possible, but unusual, to have appendicitis with a normal WBC.
