So: Appendicitis pain fever develop. Left side could be sign of proctitis, or diverticulitis. Go see gi.

Rectal pain: Two diagnoses: pelvic inflammatory disease, if sexually active without condoms. Spontaneous bacterial peritonitis. The other possibility is pelvic abscess from complicated appendicitis, maybe diverticulitis. Given age and gender, my diagnosis would be pid.
