Conventional: recommendations are to do nothing for an INR greater than ten if there is no bleeding. If there is bleeding then Vitamin K is indicated. Holding your VKA (Vitamin K antagonist) and retesting is appropriate. What will be important is to figure out why your INR became so out of control so you can do a better job of staying in therapeutic range in the future.
