Varies: The dosing of warfarin is based in part on the degree of anticoagulation that is trying to be achieved, which is different for different conditions warfarin is a blood thinner and the dosing is usually determined by measuring the prothrombin time and the inr to measure how thin the blood is becoming. Depends: For dvt? For heart valve? Prothrombin time/ inr monitoring. Inr 2.5 -3.5 is recommended. Dosage can vary and that is why tested.
