Oral allergy syndrom: Your problem is called OAS which usually does not lead to throat closure. The reason why the reaction is not consistent is likely due to allergy to just a component of the orange. For example, if you are only allergic to the allergen associated with orange peel, the only time you have problem is when the orange has not been totally peeled.
