Go to the ER!: Black coffee grounds in the stool or in vomiting are signs of fairly recent bleeding, usually in the upper digestive tract. Until the cause is determined, it should be treated as an emergency. You should go to the emergency room immediately. If you are too weak or lightheaded, do not try to drive, call for an ambulance.
