Can a white spot, which has been on the skin for many years without changing, be a skin cancer?

Probably not: Skin lesions which are stable for years without changes such as increase in size, thickening, or bleeding are very unlikely to be malignant.

Handling Skin Lesion: I recommend having a full body skin check by a dermatologist performed at least once each year or more often as recommended by your healthcare professional. A "white spot" which has been present for a few years should be evaluated and, if appropriate, tested by taking a small tissue sample (biopsy) to test for the presence of skin cancer.
