Went to doc and er for loss of hearing in right ear and fluid and air leaking out. antibiotics aren't helping when should i worry ??

Probably not: Sounds like perforated ear drum. Hearing will slowly return to normal as it heals. A physician should follow up to make sure of response to antibiotic. The eardrum sometimes must be surgically repaired if it does not heal on its own.
