See answer: He may have active TB which could cause fluid to accumulate outside of his lung in the space between his lung and chest wall(pleural effusion) and cause breathlessness. Fluid in the lung would be due to a separate condition, such as CHF. Black stools indicate he's bleeding from the upper gastrointestinal system, such as a bleeding stomach ulcer.
