Perianal abscess: You are describing a perianal abscess and anorectal fistula. An infection of one of the anal "crypts" or glands that circle the anus can work its way to the perianal skin where it forms an access that eventually drains to the outside. Anal fistula: Sounds like an anal fistula. An anal gland becomes blocked and then infected creating an abscess that tunnels to the skin. Very rarely heals with medicine such as antibiotic and usually requires outpatient (In USA) surgery
