R.O lesion: Nausea for the most part is reflux of gastric content thru gastro-esoph sphincter. If persistent then some form of partial outlet obstruction of stomach present. The on and off anemia would suggest possible lesion near pylorus that is intermittently bleeding. Gastroscopy and possible biopsy of site of any path required.
