Can you get bacteremia (specifically strep pneumoniae) from picking at a sore over and over with dirty/germy hands? or by having open cuts on hands?

No measurable risk: The risk of any infection from this is very low. Slight risk for infection with common skin bacteria like staph or strep (not pneumoniae), but for sure no risk for pneumococcus (Strep pneumoniae). Why would that even be on your mind? All things considered, this sounds mostly like a psychological problem. Discuss with your doctor.

This is not the way: one gets bacteremia.It is when one has an established,significant infection(not from a little cut or superficial small wound).
