Kidney?: I am assuming that by 'not mechanical' you mean that a musculoskeletal problem has been ruled out . So, tenderness in left flank down to the low back is kidney / ureter, until otherwise proven. I would recommend you discuss a renal ultrasound with UA/CS to r/o infection, stones or blood.
