Rare: Central retinal vein engorgement is unusual and can lead to a central retinal vein occlusion. This can be a sight threatening event. Causes can be hypertension, diabetes, blood clotting disorders, cancer, masses on the orbit or brain as well as increased intracranial pressure. I am going to assume you are under medical care if this is a finding in your eye.

Not good: If you are talking about a vein inside your eye swelling, that may lead to severe vision loss. Please seek a full work up by a retinal specialist.

Swelling: Not sure what you are asking. If the retinal veins are engaged or enlarged it may mean pending / partial central retinal vein occlusion.
