Systemic disease: A swollen retinal vein can be associated with systemic hypertension (high blood pressure), swelling of the optic nerve, or some form of obstruction preventing the blood from draining properly from out of your eye. Your ophthalmologist may need to order additional tests.

Can be normal: Are you sure it was not the optic nerve that was swollen. A dilated retinal vein can be normal (congenital). It also may indicate a compression of the draining vein. If no retinal hemorrhages are present the eye should be followed no treatment is needed. If all the veins are dilated or hemorrhages are seen a systemic workup should be done.
