Fever + stiff neck: will almost always require a lumbar puncture to rule out meningitis. Diarrhea could denote viral illness, and no confusion is a good sign. Either way, you need to get to an ED to rule out meningitis then get proper diagnosis and treatment. It does sound more viral than bacterial.
