Mgt: It may be a cluster migraine, but other causes should be considered. Your PCP may order a head MRI. If he is currently ill, he can be seen in the ED. Please check his temperature and report any fever.

Fever and HA: Fevers are not a feature of typical migraine headaches. Fever is a common feature of some kind of infection like virus or bacterial. However, an infectious headache would not be intermittent, it would be expected to be more or less constant. This should be evaluated and may need some special testing. Best wishes.
