Not overdose: 50 mg of Benadryl (diphenhydramine) is a standard adult dose; taking 50 mg more six hours later would be appropriate, so 25 mg after 4 hours isn't usually a problem. If the symptoms are progressing from rash to foot swelling, he should see a doctor.
