Not always: Depends on your symptoms but if you wait too long it makes it harder to repair later.

NO: I would recommend a comprehensive physical therapy program in conjunction with avoidance of provacative activity, nsaids and possibly a corticosteroid injection. If this fails after several months then i would consider a surgical solution.
